Medimobile Privacy Policy As a health care provider, Medimobile is bound by the Privacy Act 1988. The provision of quality health care is our principle concern. It requires a doctor-patient relationship of trust and confidentiality. Your doctor regards patient health information as confidential and will only collect this information with your consent. The main reason we collect information from you is so we can assess, diagnose and treat your illness properly and be pro-active in your health care. We also endeavor to provide recommendations to your employer for overall workplace health and safety improvements and initiatives. Your personal information is handled in accordance with Medimobile's privacy policy and consistent with the privacy legislation. Patients are entitled to know what personal information is held about them; how and under what circumstances they may have access to it; why it is held; to whom and under what circumstances it may be disclosed; when consent is required for these purposes; and how it is stored. Every effort will be made to discuss these matters with you at the time personal information is collected. Because there will be occasions when it is not practicable to make you aware of these matters at the time of collection, this document is designed to outline how Medimobile endeavors to protect the privacy of your personal health information. Collection, Use and Disclosure of Patient Information Information about your medical and family health history is often needed to provide accurate medical diagnoses and appropriate treatment. We will be fair in the way we collect information about you. Personal information is generally collected from you with your consent. However, from time to time we may receive patient information from others. When this occurs we will, wherever possible, make sure you know we have received this information. We will use the information you provide in the following ways: 1. Administration of this medical practice 2. Invoicing 3. Disclosure to others involved in your health care, including doctors and specialists outside this practice who may become involved in treating you. This may occur through referral to other doctors, or for medical tests and in the reports or results returned to us following the referrals. If necessary, we will discuss this with you. Medical care requires full knowledge of patient health information by all members of a medical team. To ensure quality and continuity of patient care your health information has to be shared with other health care providers from time to time. 4. Disclosure to others for medial defense purposes if necessary. Medimobile doctors are members of various professional bodies including medical defense organisations. There may be occasions when disclosure of patient information is required for medical defense purposes. 5. There are also circumstances where a medical practitioner is legally bound to disclose personal information. An example of this is the mandatory reporting of communicable diseases. 6. Medimobile will provide accumulated statistical data on large groups of employees (greater than 50) to their employer only in circumstances where it is impossible to identify individuals. An example is an annual health assessment where the existence of a condition found to be prevalent among the staff e.g. high lead exposure is reported to an employer as a percentage only. Data is de-identified by Medimobile only reporting statistics. The purpose of the provision of such data is to provide recommendations to your employer for overall workplace health and safety improvements and initiatives. It is necessary for us to keep your information after their last attendance for as along as it is required by law or is prudent having regard to administrative requirements. Patient information will be stored securely at the Medimobile head office and at the medical practice of the doctor, acting on behalf of Medimobile, who gathers personal information from you. Access As a patient you have a right to access your information. You may ask to view the information or ask for a copy of a part or whole record. While not required to give reasons for your request, you may be asked to clarify the scope of the request. There are some circumstances in which access may be denied but in such an event, you will be advised of the reason. A charge may be payable where Medimobile incurs costs in providing access. This will depend on the nature of the access. The material over which Medimobile and/or a doctor acting on behalf of Medimobile has copyright might be subject to conditions that prevent further copying or publication without the permission of Medimobile or the doctor. If you find that the information held about you is not accurate or complete, you may have that information amended accordingly. Upon your request, your health information held by Medimobile or its agents will be made available to another health service provider. Complaints It is important to us that your expectations about the way in which we handle personal information is the same as ours. We encourage you to discuss any concerns, questions or complaints about any issues related to the privacy of your personal information with you Medimobile doctor or Medimobile representative.
